In mental illness claims, the most important medical evidence you can provide is proof of mental health treatment. Most mental illnesses require a documented history of the illness for a minimum of two years, as well as physician statements and evidence of your ability to function in a work setting, focus, concentrate and get along in work situations with co-workers, the public and supervisors. For SSI children's disability case, SSA looks at what the teachers and doctors say about how the child functions in the home and in school. SSA wants to know what special help the child is getting through an IEP or individual educational plan or through their doctor.

Clinical depression symptoms include feelings of hopelessness, worthlessness, sadness, exhaustion, loss of interest in activities, difficulty concentrating or making decisions, weight changes, suicidal thoughts and sleep disorders. You must provide a documented medical history of a minimum of two years of depression.

Bipolar disorder (sometimes called manic depression) causes the person to cycle between manic states (mania) and depressive states. Manic states can cause feelings of euphoria as well as anger and impaired judgment. The depressive state causes the same type of symptoms as clinical depression.

Post-traumatic stress disorder can cause panic attacks, flashbacks to the trauma, fear of event recurrence, avoidant behavior and restlessness. You meet the SSA's anxiety disorder criteria and provide evidence of trouble concentrating, periods of decompensation or restriction of daily activities. Your medical records should describe a doctor's observation of your anxiety reaction.

Schizophrenia can cause a person to have trouble with social interactions, be unable to concentrate and suffer from incoherent thought patterns. Severe cases include paranoia, catatonia, delusions and hallucinations. You must provide a documented history of schizophrenia for a minimum of two years, as well as statements and evidence of the effect on your ability to hold a job.
